© Georg Thieme Verlag KG Stuttgart · New York.In the last decade, a new direction of phototherapy - the non-selective chromotherapy (NSC) with Bioptron - has become widespread in clinical physiotherapy. The aim of the study was the scientific justification of the comprehensive treatment of patients with osteoarthritis of knee joints, including the combined use of refined naphthalan applications and irradiation with incoherent polarized Bioptron light. The article presents the results of the study which investigated the efficacy of the comprehensive treatment of osteoarthritis of knee joints in patients using the combined exposure to naphthalan applications with irradiation using the non-selective Bioptron chromotherapy in the sanatorium. 64 patients aged 40-69 years with stage 1-3 knee osteoarthritis were examined and treated. It was shown that the combined use of naphthalan and non-selective chromotherapy in patients with gonarthrosis reduces pain, improves the functional state of the affected knee joints and improves the quality of life of patients in the absence of adverse effects. A comparative analysis of the study results showed that the efficacy of the combined use of naphthalan and non-selective chromotherapy is superior to the isolated effect of naphthalan. © Georg Thieme Verlag KG Stuttgart · New York.Platelet activation and aggregation have a pivotal role in arterial thrombosis and in the pathogenesis of both acute coronary syndromes (ACS) and in the thrombotic complications that occur in patients undergoing percutaneous coronary intervention (PCI). The past 30 years has seen the progress from early trials of clopidogrel and glycoprotein IIb/IIIa inhibitors to the application of more potent P2Y12 inhibitors prasugrel and ticagrelor. Early enthusiasm for newer and more potent antiplatelet agents, which could reduce ischemic events, has led to the understanding of the importance of bleeding and a desire to individualize and optimize treatment. It has increasingly become apparent that the potency and duration of dual antiplatelet therapy (DAPT) has to reflect the balance between ischemic and bleeding risk. Recently, multiple strategies have been proposed to individualize DAPT intensity and duration to reduce the bleeding and ischemic risks. Strategies of de-escalation of DAPT intensity, as well as shorter (less than a year) or more prolonged (beyond a year) treatment have been proposed, as well as platelet function test and genotype guidance of P2Y12 inhibitor therapy. Herein, we provide an overview of the progress in the field of antiplatelet therapy for ACS and PCI over the years, showing the current directions of travel. Ongoing studies focusing on personalized antiplatelet treatment will hopefully yield further insight into ways of optimizing outcomes for the individual. The proposed surgical options are as follows (1) staged procedure with elephant trunk (ET) technique, (2) traditional frozen elephant trunk (FET) intervention, and (3) beating heart cerebral vessel debranching followed by thoracic endovascular aortic repair (TEVAR). However, a marked enlargement of the proximal descending thoracic aorta might make it difficult to perform FET/ET intervention. Furthermore, because in conventional surgery for AAD, a prosthetic graft replacement is generally limited to the ascending aorta, and in repeat surgery, this short Dacron graft rarely provides enough room to allow a beating heart cerebral vessel debranching and obtaining a reliable landing zone for the implantation of a firmly anchored stent graft. Streptococcus equi subspecies zooepidemicus (SESZ) was isolated from patient's blood and two isolated mycotic aneurysms were found. The first was located in the distal abdominal aorta and the second in the right common iliac artery. Treatment consisted of penicillin administration for 6 weeks and surgical debridement of the infected tissue combined with autologous vein reconstruction. The patient was infection-free without complaints 1 year after discharge and the venous reconstruction was patent. Reports in literature of bacteremia with SESZ leading to the development of mycotic aneurysms are very scarce and show that prognosis is generally unfavorable. Canonical pathways of thyroid regulation comprise thyroid stimulating hormone-triggered receptor signaling. see more As part of non-canonical regulation, we hypothesized an interplay between protease-mediated thyroglobulin processing and thyroid hormone release into the circulation by means of thyroid hormone transporters like Mct8. We proposed a sensing mechanism by different thyroid hormone transporters, present in specific subcellular locations of thyroid epithelial cells, selectively monitoring individual steps of thyroglobulin processing, and thus, the cellular thyroid hormone status. Indeed, we found that proteases and thyroid hormone transporters are functionally inter-connected, however, in a counter-intuitive manner fostering self-thyrotoxicity in particular in Mct8- and/or Mct10-deficient mice. Furthermore, the possible role of the G protein-coupled receptor Taar1 is discussed, because we detected Taar1 at cilia of the apical plasma membrane of thyrocytes in vitro and in situ. Eventually, through pheno-typing Taar1-deficient mice, we identified a co-regulatory role of Taar1 and the thyroid stimulating hormone receptors. Recently, we showed that inhibition of thyroglobulin-processing enzymes results in disappearance of cilia from the apical pole of thyrocytes, while Taar1 is re-located to the endoplasmic reticulum. This pathway features a connection between thyrotropin-stimulated secretion of proteases into the thyroid follicle lumen and substrate-mediated self-assisted control of initially peri-cellular thyroglobulin processing, before its reinternalization by endocytosis, followed by extensive endo-lysosomal liberation of thyroid hormones, which are then released from thyroid follicles by means of thyroid hormone transporters. © Georg Thieme Verlag KG Stuttgart · New York.see more
