Tato zjištění naznačují, že na pracovištích by měly být důkladně zaváděny systémy monitorování a řízení bezpečnosti a že pracovníci by měli před i po jídle dbát zvýšené opatrnosti.We report a case of 81-year old female with Merkel cell carcinoma of skin with squamous differentiation and aberrant immunohistochemical profile. Primary skin tumor of left preauricular area was incompletely removed 6 months ago and diagnosed as poorly differentiated squamous cell carcinoma. After 6 months patient returns with relapse and new biopsy is made. Microscopically, mostly solid structures of poorly differentiated neoplasia with focal squamous differentiated are found. Neoplastic cells were small, polygonal, with scant cytoplasm and irregular oval dark nuclei with high mitotic activity. Immunohistochemically, tumor cells showed positivity for AE1/AE3, p63, CD56, chromogranin A, synaptophysin and in squamous parts p40. CK20 and other markers examined were negative. Merkel cell carcinoma is rare and highly aggressive skin tumor with characteristic expression of epithelial and neuroendocrine markers. In this report we want to point out that caution should be taken in excluding diagnosis of Merkel cell carcinoma from differential diagnosis based on atypical histological and immunohistochemical findings, especially the risk of confusion with less aggressive skin tumors.Adenoid cystic carcinoma (ACC) is one of the most common salivary gland malignancies. In rare cases, ACC undergoes high-grade transformation, which is associated with poor prognosis, in contrast to relatively long survival in the conventional ACC. Conventional ACC is characterized by typical histopathology showing glandular arrangement with sharply demarcated lumina, the tumor cells have sparse cytoplasm and angulated hyperchromatic nuclei. ACCs undergoing high-grade transformation lack these morphological features. In this paper we present a case of 46 years old female patient presenting with locally advanced tumor of the parotid gland and neck lymphadenopathy, coming for surgery. A suspect lymph node was sent to freeze section histology. Large non-cohesive cells with vesicular nuclei and prominent nucleoli along with well persevered lymph node architecture were seen in the frozen slide. This finding lead to suspicion of a lymphoma, the surgery finished in the extent of superficial parotidectomy and selective neck dissection of regions II-IV. Subsequent histopathological examination of formalin-fixed lymph node proofed epithelial nature of the atypical cells by p63 positivity. In the parotid gland resection specimen, an ACC with high-grade component was indentified. The high-grade ACC shared cell morphology with the lymph node metastasis. 17 from 20 lymph neck nodes contained metastases of high-grade ACC. Interestingly, there was strong CD117 expression in the high-grade ACC, whereas the conventional part was fully negative. It is a tumorous lesion usually caused by hemostatic material used in surgery. Such lesions are most commonly described after abdominal surgery. In this case report, the authors describe a case of a 17 year old female patient, operated for a mandible tumor. Histopathologically it was an ameloblastoma. The patient was treated lege artis, with the use of Surgicel® felt (Surgicel FibrillarTM Absorbable Hemostat). After two months, the young woman returned to clinics with a tumorous lesion at the same location. On the CT scan the lesion appeared to be a recurrence of the originally diagnosed ameloblastoma. Histopathologically, the lesion consisted of a foreign material with surrounding granulation tissue and massive inflamation. The foreign material had an atypical structure.
